Your medical declaration, liability release and consent forms. These must be signed before you can start in water training.

How it works

Signed once, held for you

  • Forms are signed on your phone or computer, from your own account. Your liability release comes first.
  • Students under 18 need a parent or guardian to complete and sign.
  • If no course shows yet, your centre has not allocated one to you. Contact the centre that took your booking.
  • Answer the medical questions honestly. It is there to keep you safe in the water, not to keep you out of it.
